Over 40 years of combined hospice experience, we are a dedicated team.

 

 

 

RN Case Managers: Oversee and manage the overall care of our patients from time of admission to discharge.

 

 

LPN Case Coordinators: Assist the Registered Nurses in providing quality care to the patients and their loved ones.

 

 

Social Workers: Qualified counselor who will assist the patients and families with the end of life decisions that may include financial services, funeral planning and other outside resources as needed.

 

 

Hospice Chaplains: Assist our patients and caregivers with spiritual needs as necessary, but will also lend a helpful hand in certain family situations. Will also consult with your pastor for your specific support needs.

 

 

Certified Nursing Assistants: Our hospice aides are all certified and assume the role in assisting the patient with personal care needs for example, bathing, showering, and other duties as designed in your individualized plan of care.

 

 

Nurse Practitioners: Qualified NP's visit the patients in the home for specialized clinical needs as well as identifying continued need for hospice services.

 

 

Volunteer Coordinators: Responsible for assigning patient's a volunteer to meet their individualized needs. The VC is responsible for recruiting and training new volunteers to help augment the care of our patients by providing brief periods of respite, reading to our patients and provide companionship.

 

 

Bereavement Counselors: Provide qualified services for bereavement and grief counseling during and after the patient's final act of living.

 

Directs our Grief Support Group Meetings held periodically at our hospice office. *Free and open to the community at large.

 

 

Administrative/Clerical Staff: The clerical staff in our hospice office offer a friendly voice on the telephone and assist you with your billing and medical records needs.
