Assisted Or Independent Living

Bridgeway Healthcare and Hospice works with Assisted Living Communities in our surrounding area. So if your loved one lives at an Assisted Living, an Independent Living, or an apartment we are able to provide care to them.

Nursing Or Skilled Care Home

We work closely with many skilled care facilities and nursing homes. Bridgeway Healthcare and Hospice is able to take care of your loved one inside of most facilities.

In-Home Care

Specialized care is provided in-home to meet each patient's individualized need.

Inpatient Care

Bridgeway Healthcare and Hospice is contracted with local Inpatient Facilities to provide that higher level of care that is often needed at the end of life.

Respite Care

Respite care provides short-term assistance for primary caregivers. Bridgeway is individually contracted with several local facilities to provide the caregiver brief relief from the daily responsibilities of their loved ones.

Volunteer Program

Our volunteers at Bridgeway Healthcare and Hospice spend time with your loved one by helping them with errands, picking up groceries, playing music, or just providing them with love and support.

Veterans Program

Bridgeway provides the best possible care to all veterans with recognition of their service to our Nation with the help of the National Hospice and Palliative Care Organization and the Veteran’s Administration.

Memory Bear Program

Our volunteers create bears using deceased loved one's favorite clothing to help the bereaved recall happy memories with their loved ones.

Grief Support Program

Bridgeway Healthcare and Hospice have specialized, trained staff available to assist you with sorting out your feelings In the weeks and months following the loss of a loved one.

Our Comprehensive Services

Expert Nursing Care

Bridgeway's Experienced Nurses and Certified Nursing Assistants are available for your loved one 24/7/365.

Physician Care

Bridgeway Healthcare and Hospice have physicians and nurse practitioners on-call 24/7/365 who provide care for your loved ones.

Dietary Care

Bridgeway's registered dieticians provide nutritional support services to clients and caregivers in the home. We conduct complete home nutritional assessments of patients and provide counseling and education regarding nutrition and dietary management.

Counseling

Bridgeway Healthcare and Hospice employ qualified spiritual and social counselors who will assist the patient and family with the end of life decisions to include financial, funeral, spiritual, and other resources.

Bereavement Services

Bridgeway Healthcare and Hospice provides free counseling for family members who need it after the loss of their loved one. We have support groups each month.

Durable Medical Equipment

Medical equipment such as wheelchairs, walkers, or oxygen is available to meet the comfort and individualized care needs of our patients.

Medical Supplies

Comfort care is important to us. Medical supplies necessary for the management of incontinence and palliative wound care are available, as related to the hospice diagnosis.

Medications & Pharmacy

As part of the hospice plan of care, medications related to the primary hospice diagnosis including those for pain and symptom management are covered under the hospice benefit, as approved by our clinical director.

I used to think of hospice as a place where people go to die. Now when I hear the word "HOSPICE" I think of many positive things, but most of all, I think of the gentle guides who eased me through the deaths of both of my parents.
