Our Mission

Bridgeway Healthcare and Hospice is a full service, licensed, and certified hospice company established in 2012. Bridgeway is unique in that it is locally owned and operated in Baton Rouge and Lafayette by hospice nurses who have over 25 years of experience. Our team is committed to patien​t care and they have an extraordinary passion for hospice.

Bridgeway Healthcare and Hospice - our team members focus on the individualized needs of the patient and their loved ones, offering them several choices.

We provide services in: Homes, Nursing Facilities, Assisted Living    Facilities, or Independent Living Facilities. 

Our Services

Hospice is a program of care for terminally ill persons which helps families take care of their loved ones at home, allowing that loved one to pass peacefully, with dignity, among family and friends. Many people feel anxious about caring for a terminally ill person at home. You may have no experience in this kind of personal care, or perhaps you have worked in the medical field but find the care a challenge. Our hospice program was developed to assist and support patients and families through this experience.

  

Bridgeway offers many services which include, but are not limited to:

Expert Nursing Care 

Nurses and Certified Nursing Assistants come out to help your family member.  Our staff is on 24/7 and we are there when you need us.  We do have agreements with nursing homes and assisted living facilities throughout the service area, if needed.  


Medical Supplies 

Comfort care is important to us. Medical supplies necessary for the management of incontinence and palliative wound care are available, as related to the hospice diagnosis.

Medications & Pharmacy 

As part of the hospice plan of care, medications related to the primary hospice diagnosis including those for pain and symptom management  are covered under the hospice benefit, as approved by our clinical director.

Medical Durable Equipment

Medical equipment, such as wheelchairs, walkers, or oxygen are available to meet the comfort and individualized care needs of our patients.  

Physician Services & Nurse Practitioners 

We have physicians and nurse practitioners who provide care for your loved one and on-call 24/7.  

Therapy Service 

Physical, Occupational, and Speech 

*Most hospices do not provide these services. 

Spiritual Support & Psychosocial 

We employ qualified spiritual and social counselors who will assist the patient and family with end of life decisions to include financial, funeral, spiritual, and other resources

 

Registered Dietitians 

Provides nutritional support services to clients and caregivers in the home. Conducts complete home nutritional assessments of patients. Provides counseling and education to patient and/or caregiver regarding nutrition and dietary management.

Volunteer Support 

We love our volunteers and they do so much to help your loved one.    Our volunteers spend time with your loved one by helping them with errands, pick up groceries, playing music, or just providing them with love and support. 

Bereavement Counseling 

We provide free counseling for family members who need it after the loss of their loved one.   We have support groups each month.  

General Inpatient Care 

At Bridgeway, we are individually contracted with several local Inpatient Facilities to provide that higher level of care that is often needed at the end of life.    

Respite Care  

At Bridgeway, we are individually contracted with several local facilities to provide a brief relief for the family from daily care of the patient.      

Donating Helps Others

Your donation goes to assisting others with memorial expenses and helping out families who are in need.
